How Do I Apply?

Check your eligibility

You may qualify for assistance through Teva Cares, if you reside in the United States, meet insurance and income requirements, and have a valid prescription for a Teva medicine that is covered under the Program.

Green check icon U.S. Residency – You must currently reside in the United States or its territories.
Green check icon Insurance – You must provide information about any healthcare insurance or prescription drug coverage plans you may have. If you do not have insurance or prescription drug coverage, please indicate that on your application.

Income – Effective January 31, 2025, all new patients applying to the PAP for 2025 enrollment must meet the new income eligibility requirement with a total pre-tax household income at or below 300% of the federal poverty level (FPL).

Most current patients are eligible to re-enroll for the same product in 2025 with an enrollment end date of 12/31/2025 provided they continue to meet the prior FPL and insurance guidelines Current patients re-enrolling on or after Nov 1, 2025, will follow new FPL and insurance guidelines.

For 2026 enrollment, all patients will be assessed under the new FPL and insurance guidelines.

Green check icon Valid prescription – You must be receiving treatment or have a valid prescription from a licensed healthcare provider practicing in the United States.
Green check icon LIS/Extra Help – Patients whose household income is less than 150% of the Federal Poverty Level (FPL) must submit proof of denial from the Low-Income Subsidy (LIS/Extra Help) program before they can be considered for assistance from Teva Cares.
