How Do I Apply?

You may qualify for assistance through Teva Cares, if you reside in the United States, meet insurance and income requirements, and have a valid prescription for a Teva medicine that is covered under the Program.

U.S. Residency – You must currently reside in the United States or its territories. |

Insurance – You must provide information about any healthcare insurance or prescription drug coverage plans you may have. If you do not have insurance or prescription drug coverage, please indicate that on your application. |

Income – Your household income must be within certain requirements to qualify for assistance. As part of your application, you will be asked to provide information about your annual gross income and the number of people in your household (includes spouse and dependents). |

Valid prescription – You must be receiving treatment or have a valid prescription from a licensed healthcare provider practicing in the United States. |
